Please wait at least two weeks from the date of purchase before you submit a request for your puppy’s registration papers. Requesting for your registration papers too early can result in a delay or system rejection due to error. If you have any questions regarding registration papers, please send an email to firstname.lastname@example.org.
Note: Hybrids/Mixed Breeds do not come with registration papers.
How to get your Puppy’s Registration Papers!
1. MAILED: If you do not reside in Las Vegas, Henderson or it’s surrounding areas, please fill out of the form below to have your puppy’s registration papers mailed directly to you. Provide your mailing address in the form provided below. Remember to double check your address before submitting it to us for any errors. We will not be held accountable for registration papers mailed to an incorrect address. Processing time can take up to 30 business days plus mail time.
2. PICK UP: If you reside in Las Vegas, Henderson or it’s surrounding areas, registration papers must be picked up at Prince and Princess Pet Shop. Sorry no exceptions. The buyer must first submit this form to request to have the registration papers ready for pick up at the store. Please give us at least 14 business days from the date of request to process your papers. We encourage you to call the store first to confirm your registration papers are ready before pick up. Be sure to mark the check box below to pick them up directly at the store for locals.
We are not responsible for mail that is lost through USPS.
Have your Puppy Contract/Bill of Sale Ready!
This form must be completely and accurately filled out in order for you to receive your papers. If the required fields are not filled out correctly on this form your submission will not be accepted due to error. Once you receive your registration papers please keep it in a safe place. The original copy is given to you, therefore there are no other copies available.
